At Acorn we have partnered with Lancashire Care Foundation Trust to contribute to the quality of the patient’s journey bringing Mental Health and Substance Misuse expertise together.


The role will work with individuals who are currently accessing Community Health services and we want people who have passion and drive to support an individual’s journey by providing practical and emotional support with a view to freeing up service demand.
